This website provides general information and a way to contact VBHC. Talking to Nico or using a linked form does not establish services or constitute an official waiver application.
Official applications and provider choice
For NOW/COMP, use your own Individual IDD Connects account to apply, upload documents, and submit. For SOURCE, contact the appropriate screening route through ADRC guidance. Agencies determine eligibility and authorization. Provider preference is a request for consideration; authorization, staffing, and start dates are not guaranteed.
Share information appropriately
Use general questions only. Do not share patient names, diagnoses, Medicaid IDs, Social Security numbers, medical records, or completed packets through public forms, Nico, or Ava. Call our office to confirm the restricted return method and recipient.
External services and urgent needs
State portals, ADRC, Retell, Generations and linked office forms operate under their own notices and procedures. This website and Nico are not emergency services. In an emergency, call 911.
