Use general questions only. Do not share patient names, diagnoses, Medicaid IDs, Social Security numbers, medical records, or completed packets through public forms, Nico, or Ava. Call our office to confirm the restricted return method and recipient.

Prepare your referral conversation

Your selections stay on this page. They are not submitted or saved. Our team records county, requested services, and provider preference during office intake.

You retain your choice of provider. Provider preference does not guarantee authorization, staffing, or a start date.

NOW / COMP: Complete the official application

1. Create or sign in to your own Individual IDD Connects account.
2. Complete the application and upload documents requested by DBHDD in that portal.
3. Submit the application. Saving a draft does not submit it.

Call VBHC for preparation and referral assistance. Official SOURCE screening follows the applicable local SOURCE provider/program route; Georgia ADRC can help identify the correct route. The responsible program determines eligibility and authorization.

4. VBHC reviews the referral and coordinates admission documents and clinical assessment when appropriate.
